Who We Are
The Children’s Health Homes of Upstate New York, LLC (CHHUNY) was established to plan for and operate as a Health Home for Children. CHHUNY is using a model of care management based upon the belief that every family has strengths and that families are the natural and most effective drivers of their care. This model also ensures that families have ‘‘voice and choice,’’ at every stage in the care management process.
CHHUNY was founded and is guided today by several leading children’s health and human services agencies and now includes a network of more than 90 children’s and family agencies across 55 counties in New York State.
Mission
The Children’s Health Home of Upstate New York, LLC is a partnership of independent agencies working to ensure that quality family-driven, youth-guided care management is provided to children and youth in upstate New York through an experienced network of providers.
Vision
CHHUNY’s vision is that there is a clear path to a healthy and successful adulthood for all New York state children through integrated, quality services and supports that promote well-being, reducing the effects of adverse events and chronic disease.
