CHHUNY believes that members have the right to be informed, equipped, and empowered to be the primary decision-maker in their lives, as this is a key motivator in the care management process. Our care managers honor this by advocating on members’ behalf.

By working together, families and care managers can build trusting relationships to maximize efforts toward the member’s welfare.
Culturally and Linguistically Responsive
CHHUNY believes that members and families with diverse cultures and language proficiency can achieve success from the care management program.
Trauma-Informed
Trauma affects a member’s sense of self, their sense of others, and their beliefs about the world. Trauma-Informed Care means that care managers recognize the presence of trauma symptoms and acknowledge the role trauma may play
in a member’s life.
Data-Driven
With CHHUNY’s approach, care managers collect data that will help stakeholders to determine the success of the Children’s Health Home program and create meaningful strategies to improve services and health outcomes across New York State.
Community-Focused
Care managers look to connect the member to resources outside of their typical services that nurture a support network throughout their communities. Children thrive when they are able to make connection and develop relationships within their community.
Team-Based
Care managers work to build a multidisciplinary team of providers for each member, in order to best coordinate services. Through a team-based approach, all stakeholders are united in a common goal to maximize effort and impact.
