POSITION TITLE:                 QUALITY PROGRAM ASSOCIATE

PURPOSE:
 
CHHUNY is seeking a qualified Quality Program Associate who will be responsible for conducting daily activities to support the overall quality assurance and improvement activities of the Health Home under the direction of the Quality Program Manager.
 
LOCATION: ROCHESTER, NY
 
 
 
RESPONSIBILITIES:

  • Assist in the daily operations and implementation of the CHHUNY Quality Program
  • Conduct chart and process audits to ensure Health Home Care Management services provided are family-driven, youth-guided and meeting policy expectations
  • Work in conjunction with the Quality Program Manager to identify problem areas to be addressed by training, policy improvements, and workflow enhancements for the Health Home Care Managers
  • Support a network of Health Home Care Management Agencies to guide them in their internal efforts as related to the CHHUNY Quality Program policy
  • Work closely with Care Managers, Supervisors, and agency Quality staff to respond to questions, requests, and suggestions for improvement
  • Analyze Care Management Agency data relevant to the overall quality and outcomes of the program to understand Care Management Agency trends, populations served, and caseload ratios
  • Identify best practices utilized throughout service provision and work with Training to implement quality improvement plans across the network
  • Under the direction of the Quality Program Manager, work with Medicaid Managed Care Plans to conduct chart audits and case reviews
  • Support the Quality Program Manager with the collection and analysis of Medicaid Managed Care Gap in Care reports, utilization reports, and Health Home outcomes.
  • Other responsibilities as related to Health Home quality initiatives to be assigned by the Quality Program Manager ongoing
 
 
 
QUALIFICATIONS:
 
  • Bachelor’s Degree in human services or other relevant field
  • Minimum of 3 years’ experience working within a healthcare quality position or relevant position as applicable to Health Home Care Management
  • Experience working with electronic health records is required
  • Experience working within the children’s system of care is preferred
  • Proficiency in Excel is necessary
