POSITION TITLE:                 Health Home Support Specialist
REPORTS TO:                        Program Manager

CHHUNY is seeking a competent, customer service focused Health Home Support Specialist to complete activities to support the daily operations of a Children’s Health Home. 

  • Conduct various activities that support the day-to-day functioning of Health Home operations which includes but is not limited to, distributing operational reports to our downstream agencies, assisting with special projects, document management, etc.
  • Have a basic understanding of Health Home policy and procedures to manage the Help Desk ticketing system to support our network of care managers throughout New York State.
  • User access management for various systems that support Health Home work (EHR, help desk, email listserv, etc.)
  • Conduct quarterly audits of various policies such as appropriate user access to systems with PHI and CMA oversight audits for background check compliance, core service requirements, and care manager qualifications.
  • Coordinate weekly update emails to our network.
  • Assist with special projects as directed by leadership.
  • Update and manage website content (no prior experience in website management needed).
  • Support the CHHUNY Compliance Officer with record requests and exclusion checking as needed.
  • Manage administrative tasks such as, but not limited to, restocking office supplies, office access logs, greeting and escorting office visitors.
  • Assist with other Health Home program activities as necessary and applicable to your role which would include cross training in referral processing.
  • Other responsibilities assigned as relevant to this position.
  • Associate degree or higher preferred but experience will be taken into consideration.
  • Proficiency in Microsoft Office products is required.
  • Experience within a healthcare setting is preferred due to the confidentiality of the data you will be working with.
